All entry DVDs must be postmarked or delivered by hand by that date. An Entry Number will be assigned automatically to each entry and provided to you upon completion of this form. Please write this number on the DVD submitted for this entry.

Note: If you are not already an ACM member and would like to join the ACM, go to the ACM website – www.alliancecm.org – and register as a member. Please print your receipt and include it with your entry to qualify for the ACM member entry fee rate.
Choose One Category per entry. Separate DVDs must be submitted for each Entry. Programs may only be entered into one category, so you have to choose which one fits your entry the best. Duplicate entries will be eliminated any category after the first one submitted with no refund. Separate episodes of a series, those with different names and subject matter, may be entered into different categories if applicable. Mixed Non-Professional/Professional – videos to be distributed and used by Community Media Centers through ACM-NE Region Website (see contest description). NO FEE!
Entry Fee must be paid by check or credit card. If paying by check, please make check payable to ACM-NE, write the assigned entry number in the memo line, and include with your DVD. Payment for multiple entries may be made with one check.
Assume You’ve Already Won! We need the following information to engrave your winning plaque or certificate; you may credit up to one title, three names and one organization.
A clip of between 20 and 30 seconds will be shown at the awards ceremony. In an attempt to accurately reflect the content of your program, please provide a timed segment of where you would like us to take your clip. We will make every attempt to comply with your request.
Thank you for entering the 13th Annual 2009 ACM Northeast Region Video Contest! Shortly after judging, all winners will be contacted by email. If you are interested in purchasing DUPLICATE copies of your winning trophy, plaque or certificates, you will be given three weeks to do so. The information will be available on the acm-ne.org website after October 25, 2010.
